Information for Patients about Virtual Care

Many clinics at North York General Hospital (NYGH) now offer telephone and virtual appointments to care for patients' safety. NYGH has video and audio technologies for most patient visits, which avoids patients needing to come in person. Sometimes, your healthcare team may ask you to visit a hospital or other healthcare facility, if necessary, for a physical examination.

Remote Patient Monitoring Virtual Care Program 

North York General has launched a new virtual care program to protect patients and prevent hospital readmissions through remote monitoring. Other patient populations may also enroll in the Remote Patient Monitoring Program, including:

  • North York at Home (home care program)
  • Palliative Care (Freeman Centre for the Advancement of Palliative Care)

The purpose of this new virtual care application is to decrease the number of recurring visits to the Emergency Department through enhanced symptom monitoring and rapid care.

Virtual Care Survey

Information Services seeks feedback from patients who have used virtual patient care to conduct clinical appointments.
