Remote Patient Monitoring Virtual Care Program

North York General Hospital has launched a new virtual care program aimed at protecting patients and preventing hospital readmissions through remote monitoring. The new North York Toronto Health Partners (NYTHP) CareConnect program utilizes an app called Vivify, which helps monitor COVID-19 positive patients who are aged 12 and older. Other patient populations can also enroll into the Remote Patient Monitoring Program, including:

  • North York at Home (home care program)
  • Palliative Care (Freeman Centre for the Advancement of Palliative Care)

The purpose of this virtual care application is to decrease the number of recurring visits to the Emergency Department through enhanced symptom monitoring and rapid care.

** If you are a primary care physician of self-referred patient please use the referral form to register for the COVID-19 Remote Patient Monitoring program:

** Physicians and NYGH units may refer those who are not COVID positive into the program via Cerner.

This initiative received provincial funding to enhance the pandemic response through appropriate virtual care options, which are key in ensuring continuity of health services while preventing the spread of COVID-19.
