I have a concern 

We are committed to providing you with an exceptional learner environment where you feel safe and respected while you are at NYGH. If you have a concern during your placement, please consider the following actions below so we can address your concern. 

*Call 911 or seek immediate assistance from onsite security or other authorities if you are concerned about impending harm to yourself or others.

Option 1: Speak with your preceptor or supervisor
If you are comfortable, we encourage you to address the concern with the person you are working with.

Option 2: Speak with your area or professional lead
If you are comfortable, address your concern with your area or professional lead. This may include your department education lead or chief, patient care manager, professional and education leader, or clinical coordinator.

Option 3: Disclose or report mistreatment with a hospital education leader or Faculty of Medicine.
We encourage you to reach out to a designated Centre for Education Leader by using the Disclosure Form, but we can also be contacted by email. Email communications should be clearly dated and labelled “Confidential report for the attention of ...” to ensure priority review.
