Our Services

The Freeman Centre for the Advancement of Palliative Care offers comprehensive consultation and follow-up care for patients with a life-limiting illness.

Our team of palliative care physicians, advanced practice nurses, nurses, social workers, psychiatrists, occupational therapists, physiotherapists, dietitians and spiritual care providers help you with:

  • Management of pain and symptoms to improve quality of life
  • Advance Care Planning
  • Goals of care discussions
  • Connecting you and your family to community resources
  • Psychosocial and spiritual care support

These services are provided in hospital, clinic and in-home settings.

We work together with your family physician and most responsible physician (MRP).

Inpatient program

When appropriate, palliative care physician is responsible for coordination of care

 Short-term acute pain and symptom management beds
We do not have a Palliative Care Unit (PCU). When a PCU bed is needed, patients will remain at North York General Hospital until a PCU bed becomes available.

In-home outreach program for patients living with cancer

The Freeman outreach program enables adults living with advanced cancer, whose wish is to stay at home for as long as possible or die at home, to receive in-home care from a Freeman outreach physician with the support of a clinical nurse specialist and other community supports as needed. We provide:  

 Seamless care, 24 hours a day and 7 days a week
Service within the following catchment area: south of Steeles Ave., north of Lawrence Ave., west of Victoria Park Ave., and east of Bathurst St.
