Reactivation Care Centre

The NYGH Finch Site Reactivation Care Centre (RCC) is located in the East building of 555 Finch Avenue West and is the same building as the former Branson Site. This location is accessible to North York residents. It is close to Hwy 401, is accessible by public transit and less than 10 kms away from the hospital (General Site). 

The building has undergone extensive renovations to provide the right environment for our patients who will receive specialized restorative care from an interprofessional team before they transition back home, to long-term care or another appropriate care setting. The RCC is designed to support patients who no need hospital services and would benefit from restorative or rehabilitative care before transitioning home or onto their next destination. The RCC model of care includes:

  • Restorative Care
  • Rehabilitative Care

The RCC offers a comfortable and healing environment that promotes privacy, dignity and family participation.  Inpatient units include bright private and semi-private rooms, large family waiting lounges, dedicated space for recreational therapy, and spacious washrooms. A partially locked unit will help ensure safety for patients, staff and visitors. NYGH and LOFT Community Services have also partnered on a unique program, Leap of Faith Together, to support RCC patients with expressive behaviours related to dementia and other conditions.
