CONNECT Care through Partnerships

Our CONNECT pillar will help us achieve our vision by:

  • Creating new models of care delivery between our hospital and community providers that better support our most complex patients
  • Enabling collaboration of interprofessional care teams to deliver the best care to our patients and families
To achieve these goals by 2018, we will:

1.  Advance integrated care by:
  • Evaluating existing integration efforts and exploring new care models
  • Building capacity within the hospital and assessing new integration opportunities
  • Supporting advanced care planning through education and tools
  • Continuing the implementation of Health Links
2.  Enhance primary care and specialist collaboration by:
  • Improving communication between hospital-based specialists and family physicians through information, technology and peer forums

Health Links brings family physicians, nurse practitioners, specialists, hospitals, Community Care Access Centres and other community services together to improve the care of complex patients.
 
Health Links brings family physicians, nurse practitioners, specialists, hospitals, Community Care Access Centres and other community services together to improve the care of complex patients. As Care Coordinators, we are the eyes in the home. We see aspects of the patient's life very clearly, like what supports they have or don't have in place.

I know my health and life are better today because of Health Links. You know that saying "many hands make light work?" Well, I believe this is true. The progress I've made is because of all the people helping to take care of me: my family, Lauren (Care Coordinator), my doctors, and the nurses who visit me at home.
