North York General Hospital's Diabetes Centre, located at our Branson Ambulatory Care Centre, has two unique centres to meet the needs of patients and their families with diabetes: the Diabetes Education Centre (DEC) and the Centre for Complex Diabetes Care (CCDC) that serves the Central LHIN region. Both centres enable and empower patients with diabetes needs to self-manage their health to achieve the best possible outcomes in a supportive and caring environment.
A referral from a primary care provider is required for both the DEC and the CCDC.
