The Cardiac Rehabilitation Clinic provides care to adult and geriatric patients with cardiovascular disease who are recovering from a heart attack, angina, angioplasty, chronic congestive heart failure, or peripheral vascular disease. It also serves patients considered at high risk for coronary heart disease, and other cardiovascular conditions.
The objective of this outpatient program is to provide long-term comprehensive management of complex and chronic cardiac conditions.
This program is a partnership with Toronto Rehabilitation Institute and a team consisting of cardiac educators (registered nurses), cardiac rehabilitation staff and exercise leaders, dietitian and physician
Features of the program include a medical evaluation, prescribed exercise program, education on strategies to reduce risk factors, and referral and counselling in a supportive and caring environment.
Once patients complete the Cardiac Rehab Program, they are discharged with recommendations on how to continue with a healthier lifestyle.
Patients are selected for the Cardiac Rehab Program through referrals from North York General Hospital cardiologists, family physicians, the Heart Function Clinic and cardiac education nurses.
